Navi General Insurance pre-auth form, portal and email ID
Download the Navi General Insurance pre-authorisation form, send it through the navi.com portal, and attach the documents below. Navi General Insurance is a private general insurance company; the cashless decision is due within one hour under IRDAI rules.
Step by step
Check the patient's health card
Note the policy number, member ID and sum insured, and check who processes cashless: Navi General Insurance itself or a TPA named on the card. That decides where this form goes.
Take the Navi General Insurance pre-auth form
Use the Navi General Insurance format if you have it, or the standard IRDAI cashless request form, which most insurers and TPAs accept.
Fill it and attach the documents
Patient part and declaration by the patient or attendant; clinical part by the treating doctor — duration of illness, provisional diagnosis with ICD-10 code, line of treatment and a cost estimate by head. Attach the documents listed below.
Send it in time
Upload it on the Navi General Insurance provider portal (navi.com) with the documents below. Keep the reference number the portal or the reply gives you. Most policies require intimation within 24 hours of an emergency admission and 48–72 hours before a planned one.
Track the decision and the discharge
The cashless decision is due within one hour and final discharge authorisation within three hours of the hospital's request (IRDAI master circular, May 2024). Answer any query the same day.
Who handles Navi General Insurance cashless requests
Navi General Insurance is a private general insurer that sells health cover alongside motor, travel and other insurance. Retail health claims are usually handled by its own health claims team; some group (employer) policies are serviced by a TPA. Check the patient's e-card: if a TPA is named there, the pre-auth goes to that TPA; if not, it goes to Navi General Insurance.
Documents to attach
- Pre-auth form signed by the patient (or attendant) and the treating doctor
- Health card / e-card of Navi General Insurance and the patient's photo ID
- Admission advice and the doctor's consultation notes
- Investigation reports that support the diagnosis (blood work, imaging, ECG as relevant)
- Cost estimate by head — room, surgeon, anaesthetist, OT, implants, medicines
- For an accident or injury: the MLC / FIR copy, or a note on why none was made
- For a planned surgery: earlier prescriptions showing when the condition was first diagnosed
Retail policy or group policy?
A retail Navi General Insurance policy bought by the patient is usually handled by Navi General Insurance's own health claims team. A group policy from an employer may be serviced by a TPA, and may have its own rules on room rent, co-pay and maternity. The health card shows which applies.
Avoid a query
Most pre-auth queries come from three gaps: no duration of illness, no ICD-10 code, and a cost estimate given as one figure. The full field-by-field list is in how to fill a pre-auth form.
If Navi General Insurance queries or denies the pre-auth
A query is not a rejection. Reply the same day, answering only what was asked, with the supporting document attached — see how to reply to a pre-auth query.
A cashless denial is not a claim rejection. The patient can pay, then file a reimbursement claim with Navi General Insurance within the policy's time limit — the denial is often reversed when the full hospital file is seen. If a claim is finally rejected, it can be reconsidered through the grievance officer, IRDAI's Bima Bharosa and the Insurance Ombudsman.
