Why health insurance claims get rejected — and how to reverse each
Almost every rejection rests on one of ten grounds. Each has a specific answer — a document, a policy clause or an IRDAI rule. Find your ground below, then follow the escalation route: insurer's grievance officer, IRDAI on Bima Bharosa, the Insurance Ombudsman, and the consumer commission.
1. Non-disclosure of a medical condition
The insurer says a condition was not declared in the proposal. Answer: it must show you knew of the condition and that it was material. A treating-doctor certificate dating the first diagnosis after the policy began defeats it. After 60 months of continuous cover, non-disclosure cannot be raised at all except for proven fraud.
2. Pre-existing disease
Answer: check the policy start date and renewals. The pre-existing disease waiting period cannot exceed 36 months under IRDAI's 2024 master circular, and portability carries your completed waiting period across insurers.
3. Initial or specific waiting period
Answer: confirm the exact wording and dates. Accidents are not subject to the initial waiting period, and specific-disease waiting periods apply only to the conditions listed.
4. Policy exclusion
Answer: exclusions are read narrowly against the insurer. The treatment must fall squarely within the excluded wording.
5. "Not medically necessary" or "could be done as OPD"
Answer: a written note from the treating doctor on why admission was clinically required. Day-care procedures listed in the policy are covered without 24-hour admission.
6. Room-rent and proportionate deduction
Answer: check whether the entitled room category was available at admission and whether the deduction was applied to items that do not vary with room category, such as medicines and implants.
7. Late intimation or late documents
Answer: delay alone should not defeat a genuine claim where the insurer was not actually prejudiced. Explain the reason and show the claim could still be assessed.
8. Missing documents or repeated queries
Answer: send everything together with an index and ask for any remaining requirement in one list. Repeated piecemeal queries are a delay you can escalate.
9. Cashless denied
Answer: a cashless denial is not a claim rejection — you can still claim reimbursement. Insurers must decide cashless requests within an hour and final discharge authorisation within three hours.
10. Suspected fraud or misrepresentation
Answer: the insurer must prove it. Ask for the investigation report and answer each finding with records.

What is the most common reason health insurance claims are rejected?
Non-disclosure and pre-existing disease grounds are among the most common, followed by waiting periods, exclusions and "not medically necessary".
Can a rejected health insurance claim be reopened?
Yes. Through the insurer's grievance officer, IRDAI's Bima Bharosa, the Insurance Ombudsman (within one year) and the consumer commission (within two years).
Can I claim reimbursement if cashless is denied?
Yes. A cashless denial does not stop a reimbursement claim with the full documents after discharge.
