Cashless claim rejected at the hospital? What to do now

A cashless denial only means the insurer or TPA would not guarantee payment to the hospital at that moment — often for want of a document. It is not a rejection of your claim. You can pay the hospital, file a reimbursement claim within the time your policy allows, and challenge any rejection of that claim through the grievance officer, the Insurance Ombudsman or the consumer commission.

Step by step

  1. Ask for the denial in writing

Ask the hospital's insurance desk for the denial or query letter from the insurer or TPA. It names the reason — usually a missing document, a waiting period or a policy exclusion.

  2. Answer a query at once

If it is a query, not a denial, the hospital can re-submit the pre-auth with the missing document. Under IRDAI's 2024 master circular the insurer must decide a cashless request within one hour and the final discharge within three hours.

  3. Collect the full file before you leave

Original final bill with break-up, payment receipts, discharge summary, investigation reports, prescriptions, the pre-auth and denial letters, and the doctor's certificate if the denial is about the illness itself.

  4. File the reimbursement claim on time

Most policies ask for the reimbursement claim within 15 to 30 days of discharge. Mention in the claim form that cashless was denied and attach the denial.

  5. Challenge a rejection of the reimbursement claim

Grievance officer (14 days), Bima Bharosa, then the Insurance Ombudsman within one year or the consumer commission within two years.

Common reasons cashless is refused

For hospitals: stop the denial before it happens

Most cashless denials are avoidable at the reception desk. Medilock's KYPP — Know Your Patient Policy — tells the hospital and the patient, before admission, the room rent the policy pays for, the ICU and treatment limits and the likely grounds for denial. And when cashless money is approved but never paid, Medilock's Hospital Assistance desk recovers it.

Is a cashless denial the same as a claim rejection?

No. It only means the insurer would not guarantee payment to the hospital then. You can still pay and claim reimbursement under the same policy.

How long does the insurer have to approve cashless?

Under IRDAI's 2024 master circular, one hour for a cashless request and three hours for the final discharge authorisation. A longer discharge delay that adds to the hospital bill is borne by the insurer.

Can I file reimbursement after cashless is denied?

Yes, within the period your policy allows — usually 15 to 30 days from discharge. Attach the denial letter.
